It probably started it´s route towards being a curse word by being used a nonobvious word for prostitutes, then meandered onwards from there.

And i KNOW it´s primarily used as an insult in common language, but that´s because a very small minority of people overall breeds dogs.
Its primary usage is still in the meaning of a female dog.
