I tried googling, but got nothing worthwhile. I also did a search here and I got too many results (probably bad key words). If this has been asked before, I apologize and would appreciate a link to the thread. If not, hopefully someone knows something from canon (manga or otherwise) and not just fanfics.

To the extent of my knowledge, nothing about how Senshi are created is ever mentioned in the anime. In the manga I think their Star Seeds are born out of the Galaxy Cauldron along with their respective worlds.

Following the lead given by Comartemis about the Star Seeds, I found this in Wikipedia:

The manga explains that star seeds are created in the Galaxy Cauldron, a sacred place located at the center of the galaxy and guarded by Guardian Cosmos. Some star seeds grow into Sailor Crystals and are sent to planets and other celestial bodies to develop with them. Eventually, a Sailor Crystal usually takes a host, and is subsequently 'born' into a Sailor Senshi. This Sailor Senshi is then the guardian of that celestial body. Upon the death of a living being, its star seed returns to the Cauldron, and can choose to either remain there or be reborn.

The shine of a regular star seed fades when the seed is separated from its owner, who then becomes a phage, or a fake Sailor Soldier.

A horribly mutated, pitiful parody of a Sailor Senshi, you mean? That's the nature of monsters of the week in the 5th season.
Lke that Sailor Guts, the body-builder who shoot projectiles of corrosive sweat? Yuck.

A host of a regular star seed, yes. Not the host of a star seed that grew into a Sailor Crystal, which, from what I understood, any star seed has the potential to do.

Whoever the Sailor Crystal takes as its host, and there doesn't appear to be any criteria who they take, then becomes a true Sailor Senshi.

That's the reason why I said anyone can be a Sailor Senshi.

Seems more like the lottery or Russian roulette to me. Never know who will get gifted with the greater seed/crystal. Although it could be a fun reason to claim all of the NWC Cologne and Happosai were granted greater crystals even without being of the Lunar court. Just look at their strengths and abilities.
